There is no single national application form for the bon senioralny that you can download as a PDF. The Council of Ministers regulation in force since 30 September 2026 only sets out what an application must contain, and each gmina prepares the form itself, usually through its social services office. You apply in the gmina where the senior lives, and the gmina has 30 days from a complete application to decide on qualification. Before you start filling in paperwork, check in the calculator whether the senior is eligible. The legal position described here is current as of 26 October 2026.
Bon senioralny application - where to get the form (PDF)
Anyone searching for "bon senioralny application 2026 pdf" is most likely to land on forms from portals that do not belong to any gmina. Do not rely on them. The regulation contains no template, only a list of what the application must include, so each gmina designs its own form and will expect that one. There are three real sources:
- The website of your gmina or its social services office (OPS, GOPS, MOPS, MGOPS or a social services centre) and its Public Information Bulletin (BIP). Gminas that have announced an intake often post the application there as a PDF together with a personal data notice. Some hand it out only at the office.
- The Council of Ministers regulation of 29 September 2026 (Journal of Laws item 1277): the full text, including § 3, which lists what the application must contain.
- The programme page on gov.pl: Program Bonu Senioralnego on the Chancellery of the Prime Minister's site. It has no form and no list of gminas, but it gives the citizens' helpline: 222 500 151, on working days from 8:15 to 16:15.
If your gmina's website shows no sign of the bon senioralny, that does not mean nothing is happening. Call the social services office, because not every gmina has started at once, as we explain below.
What the application must contain and what to attach
Under § 3 of the regulation, the application for a bon senioralny agreement contains the senior's name, PESEL number (or, without one, the series and number of an identity document), the address where services are to be provided, contact details, the information needed to check the conditions set by the Act, a statement of the senior's unmet needs in daily life, and the signature of the senior or of their statutory representative or attorney. The documents confirming that the conditions are met are attached, including those showing the senior's average monthly income over the three months before the application.
For a pension that will most likely be the ZUS indexation decision and confirmation of the amounts paid in those months, but the exact list is set by the gmina, so look for it in your gmina's form. If a child handles the matter, they must act as an attorney or statutory representative and attach a document that confirms it. If something is missing, the gmina will call for it to be completed and give at least 7 days from delivery of the request. Without it the application is left unexamined.
Work out the income beforehand, and work it out correctly. The Act refers to the definition in the family benefits act, which counts the amount after tax and contributions, not gross, so comparing the gross figure with the 3410 zł threshold is a common source of needless giving up. How to calculate it exactly is described in our guide bon senioralny 2026: who qualifies, how much and from when.
Where and when to apply in 2026
The competent gmina is the one where the senior lives, not ZUS and not the voivodeship office. The case is usually handled by the social services office or, where one exists, the social services centre. If the senior lives at home but you are considering a move to a child in another city, the competent gmina changes after the move and the voucher agreement ends. Better to check before deciding.
The Programme started on 30 September and in 2026 covers only the fourth quarter. A gmina reports its funding needs for the following year to the voivode by 30 November. According to gov.pl, 1026 gminas, or 41.4 per cent, applied for funds for this quarter, for about 84.5 million zł out of a planned 100 million zł. The remaining gminas, possibly including yours, are not yet accepting applications or are still preparing the procedure.
The order is not accidental. The Act provides that the Programme is delivered first in gminas where there had been no public care services for seniors until now. Next come those where such services cover no more than ten people, and last those with the highest projected growth in the number and share of seniors. A resident of a large city with well developed care services can therefore meet every condition and still wait longer than a resident of a neighbouring county. The pace is set by the funding limit (100 million zł for 2026, 419.23 million zł for 2027 and 519.71 million zł for 2028), not by the number of eligible people.
What happens after you apply
Filing the application is not yet the voucher. The gmina qualifies the senior without delay and no later than 30 days from a complete application, and for that purpose may talk to the senior and the people they live with and visit them at home. Delays caused by the applicant do not count towards the deadline. Needs are treated as unmet when, because of health or age, the senior cannot manage the basic activities at home alone and has no actual carer who could provide that support. When filling in the form, therefore, describe specifically what the senior cannot manage, and say honestly who helps them today and how much.
The gmina tells you the result in writing. Those who qualify go on a list that the gmina updates at least once a month. It signs agreements within the funds it has been given, first with people with the highest level of unmet needs and, at an equal level, with the older person. From delivery of the offer the senior has 21 days to sign, or they are struck off the list. Later they can ask for requalification, for example after their health worsens.
The voucher itself is services, not money: help with daily activities, hygiene and nursing care, support in using health services, and contact with the surroundings. They are free for the senior and worth up to 2500 zł a month. The agreement runs at most to the end of the calendar year for which the gmina has funds.
Who will not get the voucher even with an application
The voucher is not available, among others, to a person for whom a nursing benefit, special care allowance or carer's allowance has been granted, or who receives the supporting benefit. It is also ruled out by round-the-clock care of any kind, in a social welfare home, in a care and treatment facility or in a private care home, and by institutional day care. The exception is an ad hoc stay lasting no more than 14 days at a time. We develop this in the article on whether the bon senioralny can be combined with a care home.
The long-term care coordinator: free advice in your county
The same Act introduced a long-term care coordinator in every county. By law this is the head of the county family assistance centre or, in cities with county rights, the head of the municipal social services office or the director of the social services centre. Their tasks include individual advice on choosing long-term care services and benefits suited to a particular person's situation. At first the quality of this help will vary widely, because coordinators have until the end of 2026 to prepare, but the phone call costs nothing.
What to ask at the social services office
One well prepared phone call saves weeks of waiting. Ask directly:
- Does the gmina take part in the Senior Voucher Programme in the fourth quarter of 2026, and is it already accepting applications?
- Where is the application form and how can it be submitted?
- Which income documents are needed, and from which months?
- For how many seniors does the gmina have funds, and how long is the wait for an agreement?
- Who is the long-term care coordinator in the county?
- What care services does the gmina offer regardless of the voucher, and on what payment terms?
The last question matters more than it seems. Ordinary care services under the social assistance act have existed for a long time and in many gminas will be available sooner than the voucher. Just remember that using services subsidised from government programmes rules out the voucher, so it is a choice, not an addition.
Frequently asked questions
Is there an official bon senioralny application form as a PDF?
There is no single national template. The regulation of 29 September 2026 only lists the elements of the application, and the form, often as a PDF, is provided by the gmina on its website, in its BIP or at the social services office.
Where do I submit the bon senioralny application?
In the gmina where the senior lives, usually at the social services office or the social services centre. Check your gmina's website to see whether you can submit in person, by post or electronically.
How long does it take to get a decision on the bon senioralny?
Qualification takes at most 30 days from a complete application, and the result arrives in writing. The agreement and the start of services, however, depend on available funds and your place on the gmina's list.
Does filing an application guarantee the bon senioralny?
No. The gmina signs agreements within the funds it has been given and first with people with the greatest unmet needs, so a qualified senior may wait for an offer of an agreement.
What next: from the voucher to the cost of care and choosing a home
The voucher is a few hours of help at home, and not immediately. To plan the rest, go through these three steps in order:
- Check whether the bon senioralny applies before you start collecting documents.
- Work out what care costs and how much of it the senior's pension will cover.
- Find a care home if the senior needs constant supervision. Round-the-clock care rules out the voucher anyway, so waiting for it gains nothing.
